The Breast Cancer Risk Assessment (BCRA) AIM form was developed by tri-service Breast Cancer Specialists. It was designed for use in both primary care and specialty clinics, providing an educational and standardized format for approaching this vitally important healthcare topic. The BCRA form was designed to be used by Women’s Health Providers, Breast Specialist, and Clinical Oncologists. It focuses on Breast Cancer Risk Assessment and serves to provide consolidated care for patients undergoing treatment. The form was tailored to give the provider the opportunity to document what is necessary during the visit, or what is required by the provides clinical protocols.
