The Behavioral Health Specialty form was designed for use in behavioral health clinics. Included are Clinical Practice Guidelines for depression, PTSD, bipolar disorder, substance use disorders, suicide assessment/management, and TBI.

The Case Management form was created to help case managers standardize documentation across the DoD. It encompasses execution and documentation of the entire case management process, resulting in more accurate coding.

The Case Management-Pediatrics form is intended for Pediatric Case Managers across the DoD MTFs and allows for comprehensive execution and standardized documentation of the Case Management process.

The Internal Behavioral Health Consultant (IBHC) is for IBHCs seeing patients for brief and focused treatment in primary care settings. It standardizes the approach to common diagnoses and treatment, including depression, anxiety, adjustment disorders, sleep issues, PTSD, and others; and provides recommendations for transferring to specialty care.

The In and Out Processing form was developed to help Air Force technicians standardize the in/out process. It provides communication between multiple organizations to reduce possible errors, scrubs family records by exception, and is now mandated for all MTFs per AF1 44-173.

The MHSPHP form has been designed for input of metric-related clinical data not captured elsewhere, such as mammograms, pap smears, colonoscopies, A1Cs and LDLs. It is currently used by all three services.

The Obstetrics & Lactation form provides users with helpful tools for outpatient antepartum care. It utilizes information from prenatal visits, PHM, problems list, flow sheet, and includes embedded support guidelines to provide a comprehensive care plan.

Four age-specific Pediatrics forms have been created for management of pediatric patients, following the American Academy of Pediatrics’ Bright Futures guidelines. The forms standardize pediatric documentation across all three services and between primary care/pediatric-specialty clinics, and improve readability of encounter notes.

The Pediatric Nursing Services form is specifically designed to standardize documentation for PCMH nurses and clinical staff who provide point-of-service care to peds patients.

The Separation History & Physical Exam form was created to capture Separation Health exams and VA exams/reviews. It facilitates the mandate for every service member having a full history and physical exam completed prior to leaving active duty.
