The Behavioral Health Specialty form was designed for use in behavioral health clinics. Included are Clinical Practice Guidelines for depression, PTSD, bipolar disorder, substance use disorders, suicide assessment/management, and TBI.
The Case Management form was created to help case managers standardize documentation across the DoD. It encompasses execution and documentation of the entire case management process, resulting in more accurate coding.
The Case Management-Pediatrics form is intended for Pediatric Case Managers across the DoD MTFs and allows for comprehensive execution and standardized documentation of the Case Management process.
The Internal Behavioral Health Consultant (IBHC) is for IBHCs seeing patients for brief and focused treatment in primary care settings. It standardizes the approach to common diagnoses and treatment, including depression, anxiety, adjustment disorders, sleep issues, PTSD, and others; and provides recommendations for transferring to specialty care.
The MHSPHP form has been designed for input of metric-related clinical data not captured elsewhere, such as mammograms, pap smears, colonoscopies, A1Cs and LDLs. It is currently used by all three services.
The TSWF Mild Traumatic Brain Injury AIM form was designed to aid PCMH clinic staff in documenting care of adult TBI patients during initial and follow-up encounters.
The Obstetrics & Lactation form provides users with helpful tools for outpatient antepartum care. It utilizes information from prenatal visits, PHM, problems list, flow sheet, and includes embedded support guidelines to provide a comprehensive care plan.
The Separation History & Physical Exam form was created to capture Separation Health exams and VA exams/reviews. It facilitates the mandate for every service member having a full history and physical exam completed prior to leaving active duty.
The Sports Medicine form standardizes care and simplifies documentation for a wide range of sports medicine visits by giving both support staff and providers in PCMH clinics the tools they need at the point of care.
The Diving Event AIM form was created to be used by Undersea Medical Officers and dive corpsmen to be used during the initial evaluation and subsequent follow up after a dive incident. This form may also be reviewed and utilized by Primary Care Managers during routine appointments.
The Flight Event AIM form was created to be used by Flight Surgeons and aviation corpsmen to be used during the initial evaluation and subsequent follow up after an aviation incident. This form may also be reviewed and utilized by Primary Care Managers during routine appointments.
The Breast Cancer Risk Assessment (BCRA) form standardizes care and simplifies documentation by giving both support staff and providers in PCMH and specialty clinics the tools they need at the point of care.
